Forms for Students

When possible, please fill out the forms on your computer, print them and sign them. Bring all forms to 331 Major Williams Hall unless otherwise indicated.

  • Add/drop a major or minor. (The department does not process these during the first week of fall and spring semesters.)

  • Foreign language waiver form. If your native language is not English, you may be able to satisfy the university's language requirement with this form.

  • Independent Study and Undergraduate Research Request Form (X974, X994)

  • Field Study and Senior Tutorial Form

  • Independent Study (Graduate) Request Form 5974

  • Course Withdrawal Form. Spring 2007. You will need your advisor's signature. Then take it to the college office.

  • Authorization to Take Courses Elsewhere Please follow the instructions at the top of the form.
